Portsmouth Public Library Application Requirements

A library card with the Scioto County Portsmouth Public Library system is free to any Ohio resident or Kentucky resident residing in Lewis or Greenup counties.  Persons of any age may apply for a library card at any library location within Scioto County. ANYONE UNDER THE AGE OF 18 must be accompanied by a responsible party who is age 18 or older (such as a parent, foster parent, legal guardian, care provider, etc.).

  1. To apply for a library card, a patron must present valid proof of ALL of the following information: FULL NAME, PHOTO, DATE OF BIRTH, and CURRENT ADDRESS.  Documentation may include a valid photo ID in addition to a form of current address verification.  Current address is proof of residency within the last 30 days at that address.

(Below are acceptable forms of photo ID, D.O.B., and current address documentation.)

  • Acceptable Forms of Valid Photo I.D.
    1. State Issued Identification or Driver’s License
    2. Concealed Weapons Permit
    3. Military Identification
    4. Offender Release Card
    5. Passport
    6. Educational or Employment I.D.
    7. Green Card/Visa
  • Acceptable Proof of Date of Birth (If D.O.B. is not on photo ID)
    1. Birth Certificate
    2. Passport
    3. Military I.D.
    4. Offender Release Card
    5. Concealed Weapons Permit
  • Proof of Current Address (Must be within the last 30 days)
    1. Postmarked Mail
    2. Utility Bill
    3. Bank Statement
    4. Pay/Public Assistance Check Stub
  • If a patron cannot present a piece of mail, or has no approved form of current address documentation they may mail themselves a card/letter
    1. PPL does not provide mail/letters/stamps for the patron
  • The same name must appear on all forms of identification
  •  

  • Patrons must complete and sign a print application to signify they understand PPL’s borrowing policies
    1. The Responsible Party must sign in order for children under the age of 18 to obtain a card as well as have Internet access privileges.
  • All new cardholders receive their permanent card at the time of the application process
    1. Patrons will have one of the following status: Adult, Juvenile, Juvenile with Internet
    2. No check-out restrictions (25 items, 5 movies on Adult Cards Only)
  • Fines accrue for all adult items or any children’s items checked out on an adult card:
    1. Books, Audiobooks, Magazines, CDs, Book Kits: $0.10 per day
    2. Movies and Interlibrary Loans: $1.00 per day
    3. Adults may not check out if they have more than $5.00 in fines or 3 overdue items
    4. Juveniles may not check out if they have more than $3.00 or 3 overdue items
